What is a proof of claim form and should I do a debt relief program to avoid bankruptcy?

I’m going through a divorce and am now thinking I may need to file for bankruptcy. However, I’d appreciate first being told about any alternatives to bankruptcy since I don’t want to harm my credit history. A friend gave me a proof of claim form and I find it a bit confusing. What should I do?
